We are an out of network provider. This means that you will need to pay for your visit when you arrive and have your insurance company reimburse you.  All new patients get a complementary free evaluation.  At SOS PHYSIO, we offer treatment plans for all our patients, based on the number of visits that your physician or physical therapist has recommended.  For physical therapy, we will take all your insurance information and print claims forms for you and then your responsibility is to mail it out! You may be able to find out in advance what the details of your insurance plan are if you call them and ask about your “out of network” benefits. Please know it is ultimately your responsibility to manage your insurance company.

Yes we do accept Medicare. Please have your birth date and Medicare number, supplemental insurance   and doctor’s prescription information ready when you call to make your first appointment, so we can check your benefits for you!

Yes we do accept workers compensation. Please have your case number and your policy number information ready when you call to make your first appointment, so we can check your benefits for you! The First Report of Injury Form should be completed when the patient first seeks treatment for a work-related illness or injury, and the physician is responsible for completing this form. Workers’ compensation insurance will have no deductible or copayment, and all providers must accept the compensation payment as payment in full. Balance billing of patient is prohibited.

Yes we do accept PIP. Please have your birth date, auto insurance and Doctor’s prescription information ready when you call to make your first appointment, so we can check your benefits for you!

Depending on your insurance, your cost/co-pay/co-insurance/out of pocket will vary. Please have this conversation with our front desk before you arrive, so there are no surprises. An initial evaluation is one hour of comprehensive evaluation and treatment. This is a full hour of one on one attention from your physical therapist!

Evaluation – The fee is $125.00 per visit.

Treatment sessions – $100$(per individual session) or $75 (package of 8 min)

Acupuncture sessions – $75$(per individual session) or $50 (package of 8 min)

Combined treatments (Physical Therapy + Acupuncture) $125(package of 8 min)

You will need a prescription for physical therapy. If you don’t have one for the first couple visits, that is fine but we suggest you get one as soon as possible.

Treatments are 1 – 1.5 hours long depending if acupuncture in included. This includes discussion, active release techniques, joint mobilizations, acupuncture and whatever other skilled treatment is indicated to help you reach your desired goals as well as therapeutic exercises. It would be beneficial to come about 30 minutes earlier to do all your exercises first, and then see your therapist at the assigned time.

On your evaluation day, it is suggested to come 30 min before, so you can fill up all the paper work. Make sure to bring your insurance card and drivers’ license so we can make a copy of it. Also please bring your prescription for physical therapy if you have one.

Clothing should be loose fitting. Gym attire is preferred (shorts, t-shirt etc), Please limit jewelry and no skin lotions.

We use a team approach. Patients are shared by 1-2 therapists at most. Each and every one of the therapist has personalized unique skills. Rehabilitation, generally progresses quicker when a multidisciplinary approach is used.

At your first visit, the physical therapist and physician will recommend how often you should come in order for you to meet your specific goals. Coming more often usually results in faster progress.  Generally results are seen after 6-8 treatments.

We accept cash, check and all major credit cards.

We suggest that all patients call 24-hour before in order change or cancel any appointment. If less than 24 hours’ notice is given, a 50$ fee will apply in a discretionary fashion.

If you are late you will have less time to be treated by the therapist because there is another person with a reserved appointment after you. If you do think you will be a little late, please call us and let us know.

No, not necessarily. In Florida, a patient can see a physical therapist for the first 30 days, or 10 visits, whichever comes first, without a prescription. Please keep in mind, your insurance company will generally want to have a prescription, in order for them to reimburse. After the initial visit, our office will mail/e-mail or fax a typed copy of your evaluation to the doctor whom you specify, and that usually encourages them to sign off on the treatment plan and refer for Therapy.

Our clinic differs from traditional physical therapy clinics due to our hands on approach. We specialize in manual therapy along with Active release techniques (ART). ART is a patented, state of the art soft tissue system/movement technique that treats problems with muscles, tendons, ligaments, fascia and nerves. Headaches, back pain, carpal tunnel syndrome, shin splints, shoulder pain, sciatica, plantar fasciitis, knee problems, and tennis elbow are just a few of the many conditions that can be resolved quickly and permanently with ART. The ART provider uses his or her hands to evaluate the texture, tightness and movement of muscles, fascia, tendons, ligaments and nerves. Abnormal tissues are treated by combining precisely directed tension with very specific patient movements. This will speed up patient’s recovery process, minimize treatment length, and maximize patient’s benefits.

In our Rehabilitation program, patients will benefit from physical therapy, occupational therapy and acupuncture. Yes, you will be given an individualized home exercise program assigned to you by your therapist. As you progress, your exercises will change to meet your current needs. Through time you will gain complete independence with postural awareness and proper body mechanics.
